Reading the legislation I think there is no way to avoid answering any question and avoid being liable for a $500 fine (two specific questions can however be answered “object”).
Not signing your form would also make you liable for a $500 fine.
I can’t figure out who would be liable for a penalty under the Act if the dwelling form isn’t filled in? I’m guessing it’s the house. š
